If you have a DOT drug or alcohol violation, a Substance Abuse Professional (SAP) evaluation is the first step in the DOT Return-to-Duty (RTD) process. A SAP’s role is to evaluate the situation, recommend education and or treatment, and determine when the employee has successfully complied with the SAP’s recommendations so the RTD process can move forward.

What to Expect

  • Schedule the initial evaluation.
  • Complete paperwork before the appointment (history, releases if needed).
  • Meet with the SAP (typically 60–90 minutes) to review: DOT violation details, substance use history, work and safety considerations, prior treatment and supports.
  • Receive a written plan with recommendations.
  • Complete the recommendations (education and/or treatment).
  • Return for follow-up once completed.
  • SAP determines compliance and reports completion status per DOT requirements.
Note: The SAP does not "clear" anyone to return to duty. The employer and Medical Review Officer (MRO) manage RTD testing and return-to-duty decisions within DOT rules.

Frequently Asked Questions

How long does the process take?

Timelines vary. The initial evaluation can often be scheduled quickly. The total RTD timeline depends on the recommended education and or treatment and how fast it is completed.

Will you communicate with my employer?

Yes, within the SAP role and with appropriate releases. Most employers and C-TPAs require SAP reports and follow-up documentation.

Can I choose my own SAP?

In many cases yes, though some employers and C-TPAs have approved provider lists. Confirm with your employer.

Do you provide treatment?

SAP services are evaluation and case management for DOT compliance. Treatment may be provided by a separate program or clinician unless explicitly offered by this practice.

Is everything confidential?

Your information is handled professionally and shared only as required for DOT compliance and with the proper releases.
